According to Stanford Medicine and the U.S. National Library of Medicine – National Institute of Health, roughly 55% of patients do not take their medication as prescribed. Overdosing, underdosing, and erratic dosing intervals can diminish drug action or cause adverse effects such as criminal behavior or increased hospital and patient care visits. This cycle is unhealthy for the participant and is far more costly to the county and state.

Healthcare companies and pharmacies have the capability and resources to package medication(s) based on daily requirements at little or no cost to the end user. Pre-packaged medication reduces preventable medication errors and eliminates the risk of missed medications or incorrect dosing. The package generally illustrates the participant’s name, date and time of day for which the medication is to be taken. Some packaging can be less or more detailed based on need for the agency.

When overseeing those who take medication, especially individuals whom you suspect may be abusing them, it is important to be able to identify and confirm adherence, diversion or abuse. Thanks to ERAM, we are able observe medication intake in real-time through our smart phone application. ERAM participants are given a locked-down smart phone side loaded with our patented software. Through our HIPAA compliant web application, approved administrators and clinicians directly remote into the participants camera to generate a therapeutic contact. The admin/clinician is able to see and speak to the participant, but the participant can only hear and speak in response.

The participant is instructed to hold the medication package up to the ERAM camera, and an admin/clinician will take a snapshot of the package and its content validating that it is the correct medication about to be consumed. The participant will then open the package in front of the camera, and continue to take the medications orally with a clear glass of water. Once the medication has been taken, the participant will then guide the ERAM camera to their mouth from which they are instructed to lift their tongue, open their cheeks and lips to confirm that the medication has not been tampered with. Any form of non-compliance is then immediately reported to the appropriate party(s).

ERAM Provides Efficiency and Eliminates Excuses:

Thanks to ERAM, those who self-report to take medication that experience transportation and employment conflicts will no longer be of concern. Our remote medication monitoring eliminates such excuses which has a direct effect on “criminal thinking”. ERAM further enhances agency and case manager efficiency by reducing the need to be on site with said participant. This can substantially lower administrative costs for ACT / MAT teams.

There are a number of different ways that we can monitor this process:

  • Pre-packaged Medications: Pharmacy’s will package medication(s) based on daily requirements. Pre-packaged medication will reduce preventable medication errors and eliminate the risk of missed medications or incorrect dosing. The package generally illustrates the client’s name, date and time of day for which the medication is to be taken. Some packaging can be less detailed based on need for the agency. The client is instructed to hold the package up to the ERAM camera, and an administrator will take a snapshot of the package validating the prescription intake. The client will then open the package in front of the camera, and continue to take the medications orally with water. Once the medication has been taken, the client will then guide the ERAM camera to their mouth from which they are instructed to lift their tongue and open their cheeks to confirm that the medication has not been tampered with.
  • Dedicated Medication Referencing: ERAM’s website application allows for it to store reference photos that are dedicated to a client’s account. Based on fill dates, quantity, etc., clients can bring their medication bottles in regularly to be referenced and stored. Photos are taken of the bottle(s)/label(s) and medication(s). During a scheduled test, the client will hold their bottle and medication up to the ERAM camera for our administrators to take snapshots. Once the prescription and medication have been confirmed, the client will then proceed with the oral intake protocol as stated above.
  • Self-Reporting / Staffing: Thanks to ERAM, many of those who have transportation and employment conflicts will no longer be of concern. Our remote medication monitoring eliminates that excuse which closes the window on criminal thinking. This also allows agencies to allocate time spent on other concerning tasks.
  • Self-Intake: Often many individuals are ordered/required to take medication without any form of accountability. The lack of intake accountability is a leading indicator of prescription abuse. ERAM is built to fill this void.

Additional Information:

  • CVS and NEHI report that medication non-adherence costs the U.S. roughly $290 billion annually
  • Non-adherence is linked to 50 percent of treatment failures and leads to nearly 125,000 deaths annually
  • It can cost the state upwards of $50,000 a year or nearly $135 a day to provide and administer medication for a mentally ill inmate. Those with more severe mental illnesses can be much higher
  • In 2010, roughly 65 percent of incarcerated adults in prisons or jails met the medical criteria for a mental illness, alcohol or drug use disorder. Inmates were seven times more likely than individuals in the community to have such a condition
